ABSTRACT

BACKGROUND: Posterolateral tibial plateau fracture is a special type of tibial plateau fracture. Because of its special anatomical location, there are many kinds of surgical approaches and implants used in the treatment in and outside China, but no consensus has been reached. OBJECTIVE: To summarize various surgical approaches and implants by analyzing the local anatomical structure and fracture types of the posterolateral tibial plateau. METHODS: The databases of Wanfang Medical Network, China National Knowledge Infrastructure and PubMed were searched by computer. Using the keywords of “tibial plateau fracture; posterolateral tibial plateau fracture; internal fixation; surgical approach” in Chinese and English. According to the inclusion and exclusion criteria, 54 related articles were finally included and summarized. RESULTS AND CONCLUSION: There are local structures of common peroneal nerve, anterior tibial artery and fibular head on the posterolateral tibial plateau, which will affect the operation. The commonly used fracture types are Schatzker, AO/ATO, three-column classification systems and posterior column 4 zone. Posterior column 4 zone has a more definite guiding effect on posterolateral tibial plateau fractures. There are many kinds of surgical approaches and internal plants for the treatment of posterolateral tibial plateau fractures, and the optimal selection should be made according to the fracture characteristics. For simple posterolateral tibial plateau fractures, the posterolateral approach has a certain advantage over other approaches. Posterolateral supporting plate has a better supporting effect on posterolateral shear fracture of tibial plateau.

ABSTRACT

Objective To explore difference of neuropsychological tests between the remitted schizophrenia patients and the first-degree non-psychotic relatives of schizophrenia patients,and to explore the possible endophenotypes in cognitive function.Methods The MATRICSTM consensus cognitive battery was administered to 86 remittent schizophrenia patients (the patient group),86 first-degree non-psychotic relatives of schizophrenia patients (the relative group),and 86 healthy people (the control group).Results Compared with the control group,the patients group and the relative group showed poor performance on trail making test,brief assessment of cognition,hopkins verbal learning,mazes,category fluency,emotional intelligence test and continuous performance test (P<0.05).There were significantly lower performances in the patient group than the relative groups on hopkins verbal learning,mazes,emotional intelligence test and continuous performance test (P<0.05).Conclusion The remitted schizophrenia patients and their first-degree non-psychotic relatives have selective deficits in cognition function,among them,verbal leaning,attention/vigilance,reasoning and problem solving and social cognition may be potential endophenotypes for schizophrenia.

ABSTRACT

Objective To observe the efficacy difference between different needling manipulations in treating acute stroke with point-toward-point needling at scalp acupoints, for seeking the optimal treatment protocol. Method Ninety patients with acute stroke were randomized into a manual needling group, an electroacupuncture group, and a needle-retaining group, 30 cases each. In addition to the neurological treatment, the three groups were all given point-toward-point needling from Baihui (GV20) to Taiyang (EX-HN5) on the affected side, respectively with manual twisting, electroacupuncture and needle retaining methods. Before and after 14-day treatment, the Neurological Deficit Score (NDS), level of neuron specific enolase (NSE) and total efficacy were compared among the three groups. Result The NDS dropped significantly in the three groups after the intervention (P<0.05); the manual needling group showed the most significant improvement, better than the electroacupuncture group and needle retaining group. The level of NSE declined significantly in the three groups after the intervention (P<0.05); the decrease in the manual needling group was the most significant compared to that in the electroacupuncture group and needle retaining group. The total effective rate in the manual needling group (90.0%) was significantly higher than that in the electroacupuncture group and needle retaining group (P<0.05). Conclusion Point-toward-point needling at scalp acupoints with manual operation can effectively improve the neurological deficit in stroke patients, and the application of needling manipulations can further enhance the therapeutic efficacy, which is specific.

AIM To establish the quality standard for Weigela japonica Thunb.var.sinica (Rehd.) Bailey (W.j.).METHODS TLC was adopted in this medicinal material's qualitative identification after morphological identification and microscopic identification.The contents of water,total ash,acid-insoluble ash and extracts were detected according to Chinese Pharmacopoeia methods.Then the contents of scopoletin and total coumarins were determined by HPLC and UV,respectively.RESULTS The morphologies and microscopic characters of W.j.could be distinguished from other same generic plants.The clear TLC spot displayed good resolution.The contents of water,total ash,acid-insoluble ash,water-soluble extract and acid-soluble extract were no more than 12.0%,no more than 2.0%,no more than 0.5%,no less than 5.0% and no less than 4.5%,respectively.Scopoletin and total coumarins showed good linear relationships within the ranges of 1.25-40.0 μg/mL(r =0.999 7)and 2.0-64.0 μg/mL (r =0.999 9),whose average recoveries were 98.19% (RSD =0.90%) and 99.21% (RSD =2.5%),respectively.CONCLUSION This accurate and reliable method can be used for the quality control of W.j..

ABSTRACT

Objective To examine the difference of neurological soft signs (NSS) among the remittent schizophrenia patients,first-degree non-psychotic relatives of schizophrenia patients and healthy controls.Methods The Cambridge Neurological Inventory (CNI) (the Chinese version) was administered to 86 remittent schizophrenia patients (patient group),86 first-degree non-psychotic relatives of schizophrenia patients (relative group) and 86 healthy controls (control group).Results Compared with relative group,NSS total-score,the subscore of motor coordination and sensory integration were significantly higher in patient group (P<0.01).Compared with control group,NSS total-score,the subscore of motor coordination and sensory integration were significantly higher in patient group (P<0.01).Compared with control group,NSS total-score and the subscore of motor coordination were significantly higher in relative group (P<0.01).Conclusions The levels of NSS in remittent schizophrenia patients and their first-degree non-psychotic relatives are higher than normal control,and the patients have more NSS than their relatives.The motor coordination nay be a potential endophenotype for schizophrenia.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the clinical significance of peripheral blood NK cell number change in patients with mantle cell lymphoma.</p><p><b>METHODS</b>Eight-four patients with mantle cell lymphoma treated in our hospital from November 2003 to November 2011 were studied, the venous blood was collected from all patients and detected with flow cytometry. The age, sex, pathologic type, B-symptoms, clinical stage, absolute NK count(ANKC), hemoglobin(Hb), lactate dehydrogenase(LDH) and β2-microglobulin(β2-MG) levels, bone marrow involuement(BMI) and the international prognostic index of mantle cell lymphoma(MIPI) were recorded. All patients were followed up.</p><p><b>RESULTS</b>There were no significant differences in ANKC among different age, sex, B symptom, Ann Arbor stages, Hb, LDH and β2-MG levels, BMI and MIPI of patients with MCL(P>0.05). The sensitivity and specificity of ANKC were 76.6% and 73.8%, respectively. The optimal throshold of ANKC was 0.10×10/L and AUC was 0.798(95% CI: 0.689-0.902)(P<0.01). The 3 year-PFS rate in patients with ANKC≥0.10×10/L was higher than that in patients with ANKC<0.10×10/L(68.2% vs 32.10%)(P<0.01). The 3 year-OS rate in patients with ANKC≥0.10×10/L was significantly higher than that in patients with ANKC<0.10×10/L (85.1% vs 43.8%)(P<0.01). Multivariate analysis showed that the independent predictors of PFS and OS in patients with MCL were ANKC and MIPI(P<0.05).</p><p><b>CONCLUSION</b>The ANKC in peripheral blood has an important value for judging the prognosis of patients with MCL and can be used as an important index to judge the disease status of patients with MCL.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To compare the clinical features in children with different control levels of asthma and to explore the factors influencing asthma control.</p><p><b>METHODS</b>A cross-sectional study was performed on 115 children diagnosed with asthma between October 2013 and February 2014. All the patients were classified into two groups: fully controlled group (n=65) and non-fully controlled group (n=55), according to the Children Bronchial Asthma Prevention and Treatment Guideline (2008 version) and the asthma control test results. The differences of clinical features between the two groups were compared. The quality of life was evaluated by an asthma-related quality of life questionnaire. The main factors influencing asthma control were analyzed by the logistic regression method.</p><p><b>RESULTS</b>There were significant differences in the frequencies of respiratory tract infection and acute asthma attacks within the 3 months, and unplanned hospital visits due to acute asthma attacks between the fully controlled and non-fully controlled groups (P<0.05). The scores of asthma-related quality of life in the fully controlled group were significantly lower than in the non-fully controlled group in children under 7 years old. In contrast, the scores of asthma-related quality of life in the fully controlled group were significantly higher than in the non-fully controlled group in children at the age of 7-16 years (P<0.05). The logistic regression analysis showed that the patients without experiencing regular hospital visits (OR=7.715) and with allergic rhinitis (OR=5.531) had increased risks for poor asthma control and that the patients with other allergic diseases (eg. eczema, food allergy) had decreased risks for poor asthma control (OR=0.299).</p><p><b>CONCLUSIONS</b>The appearance of some clinical features suggests that the asthmatic children may be in the status of poor asthma control and need an active intervention. A poor asthma control status can result in a decreased quality of life. To improve the asthma control level, the incidence of allergic rhinitis should be reduced and a regular hospital visit should be performed in the children.</p>

ABSTRACT

Objective We sought to develop Nurse Job Performance Scale and test its validity and reliability.Methods Initial version of Nurse Job Performance Scale was formulated by using literature review and qualitative interview.By cross-sectional investigating of 562 nurses in coastal cities by convenience sampling as well as screening and selecting items through item analysis,the validity and reliability was tested under the statistics.Results Three factors containing 14 items were extracted by using exploratory factor analysis:work coordination,working enthusiasm and work involvement.The average value of factor loading was between 0.643~0.839,the 3 factors explained 73.9% of the total variance.The total Cronbach's α was 0.937 and the Cronbach's α coefficients of every dimension were between 0.767~0.930.Conclusions Nurse Job Performance Scale has good validity and variability.

ABSTRACT

According to the characteristics of the etiology and pathogenesis of child cerebral palsy, on the basis of "regulating the mind in treatment of all kinds of diseases" and "regulating the functions of five zang organs with back-shu points", Professor DONG Gui-rong applied the penetrating needling technique on the scalp points and acupuncture at back-shu points of five zang organs in the treatment of child cerebral palsy. The valuable clinical experiences have been summarized as "regulating the mind with scalp needling technique, benefiting the brain and opening the orifice", "regulating five zang organs with back-shu points" and "integration of acupuncture and rehabilitation, and function reconstruction". Two effective cases were introduced.

ABSTRACT

In our previous work, we found that trivalent dimethylarsinous acid (DMA(III)) have high affinity binding to cysteine residue 13 of rat hemoglobin. However, it is still unknown why arsenic intermediate metabolite DMA(III) has high binding affinity for Cysl3 but not for other cysteine residues 93, 140, 111 and 125. In order to better understand the molecular mechanism of DMA(III) with rat hemoglobin, we have done current study. So, SD rats were divided into control and arsenic-treated groups randomly. Arsenic species in lysate of red blood cells were analyzed by HPLC-ICP-MS, and then determined by a hybrid quadrupole TOF MS. In addition, trivalent DMA(III) binds to different cysteine residues in rat hemoglobin alpha and beta chains were also simulated by Molecular Docking. Only Cys13 in alpha chain is able to bind to DMA(III) from the experiment results. Cys13 of alpha chain in rat hemoglobin is a specific binding site for DMA(III), and we found that amino acids compose pockets structure and surround Cys13 (but not other cysteine residues), make DMA(III) much easy to bind cysteine 13. Taken together, the DMA(III) specific binding to Cys13 is related to spatial structure of Cys13.

ABSTRACT

Objective To investigate the risk factors of acute kidney injury following coronary artery bypass graft(CABG).Methods Twenty-six patients suffering from acute kidney injury following CABG as the patient group and 59 individuals without impairment of renal function as the control from January 2008 to September 2012 in the First People's Hospital of Yichang according to a retrospective case-control study and nonconditional multivariable Logistic regression analysis method..Results Among the 85 patients undergoing CABG,26 suffered acute kidney injury following CABG with a morbidity rate of 30.59%.Identified risk factors for acute kidney injury following CABG included pre-operative mederale-sever cardiac insufficiency,prolonged mechanical ventilation,prolonged stay in ICU and anemia before surgery.The identified independent risk factor of acute kidney injury following CABG was pre-operative mederale-sever cardiac insufficiency (OR =3.206,95% CI:1.067-9.631).Condusion Pre-operative mederale-sever cardiac insufficiency was an independent risk factor of acute kidney injury following CABG.

ABSTRACT

Objective To investigate the role and significance of fiberoptic bronchoscopy in diagnosis of congenital airway dysplasia.Methods The clinical data of 606 children with bronchoscopy who were admitted to Department of Respiratory Medicine,Nanjing Children's Hospital Affiliated to Nanjing Medical University from Mar.2008 to Mar.2012 were retrospectively analyzed.Results Among the 606 children,airway dysplasia were found in 120 cases,duded tracheobronchomalacia in 47 cases,racheobronchial stenosis in 31 cases,tracheal bronchus in 27 cases,laryngomalacia in 9 cases,abnormalities or variations at the bronchi opening in 16 cases,airway diverticulum in 8 cases,tracheal esophageal fistula in 5 cases,and 23 children in the 120 cases had 2 or more deformity.Conclusions The fiberoptic bronchoscopy plays a decisive role in diagnosis of congenital airway dysplasia.Fiberoptic bronchoscopy should be performed in suspicious children as soon as possible to determine the cause.

ABSTRACT

Based on previous report that the Chinese herb Ligustrum lucidum (LL) extract directly inhibited hepatitis C virus (HCV) replicase (NS5B) activity, the active components of LL extract to inhibit HCV NS5B activity and their inhibition mode were investigated in this study. LL extract was separated using ethyl acetate and thin layer chromatography (TLC). The inhibitory activity of separated fractions on HCV NS5B was analyzed by the inhibitory assay of NS5B activity. The results showed that only fractions 1 and 2 inhibited NS5B activity, and fraction 2 possessed higher inhibitory activity than fraction 1. HPLC analysis combined with inhibitory assays indicated that ursolic acid and oleanolic acid are the active components within fractions 1 and 2 to inhibit NS5B activity, separately. Moreover, oleanolic acid possessed higher inhibitory activity than ursolic acid. Further inhibition mode analysis found that both oleanolic acid and ursolic acid suppressed NS5B activity as noncompetitive inhibitors. The Ki values of ursolic acid and oleanolic acid were about 4.7 microg x mL(-1) (10 micromol x kg(-1)) and 2.5 microg x mL(-1) (5.5 micromol x kg(-1)), respectively. Taken together, these results demonstrated that oleanolic acid and ursolic acid suppressed NS5B activity as noncompetitive inhibitors, implying that the two natural products have potential value for HCV therapy.

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the status of dietary intake of Chinese adults in 2010.</p><p><b>METHODS</b>In 2010, China Chronic Disease Surveillance was carried out in 31 provinces and Xinjiang Production and Construction Corps. The survey was undertaken in 162 surveillance sites in the national disease surveillance points system by stratified multi-stage cluster random sampling. The participants were local residents (living in the area for more than 6 months) aged 18 years and above in the surveillance sites. A semi-quantitative food frequency questionnaire was used to measure the dietary intake for 97 187 urban and rural adults aged 18 years and above. After complex weighting for the sample, the average daily consumption for major food was analyzed for different genders and regions.</p><p><b>RESULTS</b>After complex weighting, the median of cereal, rice and wheat flour intake for Chinese adults was 375.0 g. The median of livestock meat intake was 57.1 g with western areas (85.7 g) much higher than eastern (57.1 g) and central areas (46.2 g) (χ² = 2054.82, P < 0.01). The median of poultry intake was 7.1 g, eastern areas (10.0 g) higher than western and central areas (both 6.7 g) (χ² = 1210.86, P < 0.01). The median of seafood intake was 14.3 g, eastern (15.0 g) much higher than central (14.3 g) and western areas (7.1 g) (χ² = 5603.37, P < 0.01). The median of daily intake for eggs was 21.4 g, western (14.3 g) lower than eastern and central areas (both 21.4 g) (χ² = 1699.11, P < 0.01). The median of dairy product intake was 42.9 g, eastern (57.1 g) higher than western and central areas (both 35.7 g) (χ² = 196.35, P < 0.01). Overall, 52.8% of urban and rural residents did not meet recommended amount of fruit and vegetable intake. The rate of inadequate intake of fruit and vegetable was higher in rural (55.7%) than urban areas (46.1%, χ² = 929.43, P < 0.01).</p><p><b>CONCLUSION</b>The adult intake of cereal category foods tended to decline in 2010, and the intakes of vegetable and fruits, fishery products as well as dairy products were evidently deficient and regionally varied.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To verify the safety of penetrating acupuncture at the head points for cerebral hemorrhage at the acute stage.</p><p><b>METHODS</b>Sixty cases of cerebral hemorrhage at the acute stage were selected and randomized into a western medication group and a head-point group, 30 cases in each one. In the western medication group, the anti-cranial pressure and anti-blood pressure program was administered with the conventional intravenous infusion of Sodium Nitroprusside, Mannite, etc. In the head-point group, on the basis of the treatment as the western medication group, the penetrating acupuncture at the head points was supplemented. For consciousness disturbance, Taiyang (EX-HN 5), Benshen (GB 13) and penetrating needling from Shenting (GV 24) to Shangxing (GV 23) were selected. For headache, the penetrating needling was adopted from Shangxing (GV 23) to Yintang (EX-HN 3) and from Fengchi (GB 20) to Fengchi (GB 20). For disturbance of urination and defecation, the penetrating needling was adopted from Baihui (GV 20) to Sishencong (EX-HN 1). For aphasia, the penetrating needling was adopted from Fengfu (GV 16) to Yamen (GV 15), etc. For facial paralysis, the penetrating needling from Dicang (ST 4) to Jiache (ST 6) and Xiaguan (ST 7) were selected. For paralysis, the penetrating needling was adopted from Baihui (GV 20) to Taiyang (EX-HN 5), etc. Before and after treatment, the changes in cerebral hemorrhagic volume, the results of blood, urine and stool routine examination, the situations of the life indices such as heart, liver and kidney functions, blood pressure, respiration and heart rate were observed, as well as the impacts on ECG.</p><p><b>RESULTS</b>The cranial CT sania cn indicated that the rate of the basic and significant absorption of hematoma was 80.0% (24/30) in the head-point group, which was superior to 56.7% (17/30) in the western medication group (P < 0.05). Before and after treatment, the results of blood, urine and stool routine examination and the life indices such as blood pressure, respiration and heart rate were all in the normal scope for the patients in the head-point group (all P > 0.05). There were no any abnormal changes in liver and kidney functions. In the head-point group, the abnormality rate of ECG was lower obviously than that in the western medication group [16.7% (5/30) vs 43.3% (13/30), P < 0.05].</p><p><b>CONCLUSION</b>The penetrating acupuncture at the head points is safe in the treatment of cerebral hemorrhage at the acute stage.</p>

ABSTRACT

The case-based teaching method which is applied to the teaching of Science of Meridians, Collaterals and Acupoints is discussed in this paper, in which the typical cases such as the growth of the acupuncture-moxibustion eminent physicians, the application of acupoints by the eminent physicians and the experiences in the acupoint combination are integrated. The students are instructed to launch the clinical practical activity through establishing the clinical base of Science of Meridians, Collaterals and Acupoints, in association with the true cases. It is proved that the case-based teaching method can promote the training of the manipulation techniques of the students and the inheritance of the experiences of eminent physicians in higher education.

<p><b>OBJECTIVE</b>To observe the effect of the new Bianshi (stone needle) therapy on migraine.</p><p><b>METHODS</b>Eighty cases of migraine were treated by the new Bianshi therapy with the multi-functional fish-shaped stone board. Manipulations such as pushing, scraping and kneading at acupoints were applied on head, neck and back as well as Baihui (GV 20), Shuaigu (GB 8) and Fengchi (GB 20). The intensity of headache and clinical effect before and after the treatment were observed.</p><p><b>RESULTS</b>The pain score before the treatment was (8.31 +/- 0.61), and (3.90 +/- 0.82) after the treatment. The intensity of pain after the treatment was sighificantly lower than that before the treatment (P < 0.01). The total effective rate of the excessive syndrome [hyperactivity of liver yang and blood stagnation in meridians and collaterals, 100% (40/40)] was better than that of the deficiency syndrome [kidney yin deficiency and qi and blood deficiency, 87.5% (35/40)] (P < 0.05).</p><p><b>CONCLUSION</b>The new Bianshi therapy is applicable for treatment of migraine, which has a comparatively ideal clinical effect.</p>

ABSTRACT

<p><b>OBJECT</b>To explore the better treatment of lumbar intervertebral disc herniation.</p><p><b>METHODS</b>One hundred and thirty-two cases of lumbar intervertebral disc herniation were randomly divided into an observation group and a control group, sixty-six cases in each group. In observation group, Baihui (GV 20) penetrating to Taiyang (EX-HN 5), Jiaji (EX-B 2), Huantiao (GB 30) and Chengfu (BL 36) etc. on affect side were applied; in control group, Shenshu (BL 23), Dachangshu (BL 25), Huantiao (GB 30) and Chengfu (BL 36) etc. on affect side were applied. Once a day, 6 treatments made one session and one day break between courses. The curative effects after 4 and 8 courses and the recurrence rate that 6 months after treatment were observed; the pain scale evaluated by Visual Analogue Scale (VAS) and the Oswestry Disability Index (ODI) were observed.</p><p><b>RESULTS</b>The cured rate and total effective rate were 44.3% (27/61) and 100.0% (61/61) in observation group, 26.7% (16/60) and 93.3% (56/60) in control group after 4 courses; there were significant differences between two groups (all P < 0.05). After 8 courses, the cured rate and total effective rate were no significant differences between two groups (all P > 0.05). In observation group, the recurrence rate was 9.4% (5/53) 6 months after treatment, which was lower than that (24.1%,13/54) in control group (P < 0.05). The VAS and ODI scale were significantly reduced (all P < 0.05) after treatment, and which in observation group were inferior to those in control group (all P < 0.01).</p><p><b>CONCLUSION</b>The lumbar intervertebral disc herniation treated with puncture by opening meridian and consolidating origin is rapidly effected with low recurrence rate.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe the clinical effects of scalp penetration acupuncture and conventional scalp acupuncture for treatment of acute apoplexy, to seek the effective location and mechanism of scalp acupuncture treatment for acute apoplexy.</p><p><b>METHODS</b>Sixty cases of the apoplexy were randomly divided into an observation group and a control group, 30 cases in each group. Basis on the neurological treatment, the observation group was treated with penetration needling through Baihui (GV 20) to Taiyang (EX-HN 5) on the affected side, with four needles relay; the control group was treated with penetration needling at anterior oblique line of parietotemporal region, with four needles relay. After 14 days of treatment, the neurological deficit scores (NDS) and the therapeutic effects were compared before and after treatment, and the contents of plasma endothelins (ET) and the calcitonin gene-related peptide (CGRP) were tested respectively on the 2nd and 14th day after treatment.</p><p><b>RESULTS</b>The effective rate of 86.7% (26/30) in the observation group was superior to that of 80.0% (24/30) in the control group; the NDS of both groups were obviously decreased after treatment (both P < 0.01), the improvement degree in the observation group was better than that of the control group (P < 0.01); the ET level was obviously decreased and the CGRP level was obviously increased in both groups after treatment (all P < 0.01), and the range of change was more significant in the observation group (P < 0.01).</p><p><b>CONCLUSION</b>Penetration needling through Baihui (GV 20) to Taiyang (EX-HN 5) is an effective treatment for acute apoplexy, which is better than conventional scalp acupuncture.</p>
